I understand that upon completing this application, I will have to; complete an enrollment agreement online, provide proof of social security number, Proof of date of birth (driver’s license or birth certificate) and provide a High School diploma/Official Transcripts or equivalent, GED certificate, or home school certification.

By submitting this enrollment form and providing my payment information, I authorize the International Institute of Cosmetology (“the School”) and its payment processor (Square) to:
Charge my card or bank account today for the initial payment required for enrollment; and if applicable, automatically debit the same card or bank account for all scheduled installment payments under the payment plan I selected and continue debits until the total balance owed for the program is paid in full.

I understand and agree that: My payment method information will be stored by Square for recurring billing. Ialso understand that by selecting a payment plan:
I must complete all installment payments as scheduled. Payment plans are interest-free, but missed or late payments may result in penalties, financial holds, or termination from the program.
I must maintain a valid debit/credit card or ACH account on file throughout the entire payment plan period.
I understand and agree that, I am responsible for ensuring that sufficient funds are available in my account on each scheduled payment date.
A late fee of $20.00 may be assessed for any installment more than five (5) days late, consistent with the School’s catalog. A $25.00 returned payment/ACH fee will be charged for any failed bank draft or returned payment. If any payment fails, Square may automatically retry the payment using my saved payment method.
